During the penultimate month of your training programme you need to ensure that the Deanery receives your Completed GP Feedback questionnaire.

You will find this document in the box on the right hand side of this page.  Please complete and send this back to us as soon as possible by

It is very important that the Deanery obtains feedback on your training experience. This information may be used at your trainer’s next re-approval visit as part of an ongoing formative process unless you specify otherwise. 

Your ARCP form has been or is currently being processed by the Assessment Team.  Please check the dates and specialties of the posts on your form before you sign it and let us know if you see any discrepancies by email (eportfolio@londondeanery.ac.uk). 

Once you have electronically signed your ARCP form, you will then be able to apply online for your certification (as well as applying online to the GMC).  Please see the Certification page for further information.
