What does this cover?

The Data Protection Act 1998 requires that organisations that collect and hold personal information do so carefully.  The Act outlines 8 data handling principles that the London Deanery is committed to adhering to.  Personal information must be:

1. Fairly and lawfully processed
2. Processed for limited purposes
3. Adequate, relevant and not excessive
4. Accurate and up to date
5. Not kept for longer than is necessary
6. Processed in line with your rights
7. Secure
8. Not transferred to other countries without adequate protection

Individual rights provided by the Data Protection Act

Subject Access Requests
The Data Protection Act provides individuals with the right to know what information the London Deanery holds about them.  This process is known as making a subject access request.

What is the process for making a subject access request?
You must apply in writing, ideally by completing the Data Protection Access form that can be downloaded from this page.  You can then either send the form by post or email.  If you choose not to use the form provided please ensure that you provide sufficient information to enable us to identify the information that you are requesting.  Your application may be delayed if we need to seek clarification from you.  If other people can be identified in your personal information these references will usually be removed prior to its release to you.  Your application will be processed within 40 calendar days.

Identification
We have a duty to ensure that we only release information to the individual that is the subject of the information request.  We therefore need you to provide some identification to accompany your application:  This should be:

  • A photocopy of your passport or driver’s license and an original bank statement or utility bill for proof of address.  Original documents will be returned to you.

What are the costs?
The cost of a subject access request is £10.00.   This fee must be attached to your application.  If you do not send the fee it will delay processing of your application.  Cheques should be made out to NHS London.
